Passports are available at most U.S. Post Offices

NOTICE! You MUST make an appointment in order to be admitted and apply.

NOTE! These appointment are for NEW passports ONLY! If you need to RENEW your passport, you will find the link to the online form at the end of these instructions.

1. Click the link below to make an appointment to apply in person. http://usps.com/international/passports.htm

2. In order to apply in person, click the link below to access the online passport form - https://pptform.state.gov/PassportWizardMain.aspx

3. When the page opens, choose "Fill Out Online and Print" (this is Form DS-11 for first-time applicant or children). Click the "Submit" button to access the form on the next page.

4. Complete ALL the form online and print a copy (but do NOT sign it!)

5. Bring your UNSIGNED printed form with you to your appointment (along with the other items in steps 6-9 below).

6. You will also need to bring evidence of your U.S. Citizenship (such as an OFFICIAL birth certificate) (PLUS a photocopy of your document!).

7. You will also need proof of your identification (such as a TN Driver license with photo) (PLUS a photocopy of this ID!).

8. Bring a passport photo of yourself (available at places such a CVS, FedEx, UPS or Walgreen's store).

9. Bring a personal check or a money order (made payable to the U.S. Dept. Of State) to pay for your passport and application fees (click the link below to determine how much the money order should be). https://travel.state.gov/content/travel/en/passports/how-apply/fees.html

10. Review all the steps above to be sure you have everything that is required before you arrive for your appointment!

FOR PASSPORT RENEWALS, you will need to use the online form number DS-82 which must be printed and mailed along with a form of payment. The renewal form can be found at http://travel.state.gov/content/travel/en/passports/have-passport/renew.html
